If you are a patient searching for a psychological assessment, look no further! Follow the instructions below and you can streamline your process of getting your evaluation done.

  1. Go to this link.

  2. Create a username using your first and last name, enter an email address to which you have access, and create a password.

  3. Click on the box acknowledging that you accept the Terms and Conditions, then click on ‘Create Account’.

    4. Fill out your information in the available fields and click on ‘Save’.

  4. Scroll down and review the three consent forms displayed. Sign each of them and make sure that you sign both signatures (buttons) Authorization of Release of Information.
