Dr. Randolph is able to do several types of assessments. We have refined the system of getting you into the office, through the process, and out in the quickest way possible. We’ll make this easy for you.
The process is as such:

You sign up or your doctor sends you as a referral to our office.

We take in your information and then contact you to set up an appointment.

We’ll send some forms to your email. These are the Authorization for Use or Disclosure of Protected Health Information, the Consent to Treat, the Notice of Privacy, and the generalized intake of questions for your assessment.

When you come into the office, we’ll give you further objective testing to assure that we have an accurate view of how things are, which we will score and enter into our secured and encrypted system.

Dr. Randolph will then visit with you to go over everything and perform the assessment. Typically, the visit consists of reviewing your medical history. It also involves a social history, family history, information about previous alcohol or substance use, and what kind of psychological treatment you may have had in the past. The conversation remains confidential between you and your psychologist, except to share the information with your surgeon in the signing of a release form.
After that, you will be finished and on your way to getting what you need!
Bariatric / Weight Loss Procedures
This procedure is risky, and riskier if other issues may interfere. This includes psychologically, just like physiologically. Some of the things we will discuss are eating habits, exercise, family support, sleeping habits, etc…
When faced with the challenges of dietary restrictions, changes in habits, changes in body image, and new cognitions and feelings, you will face a new lifestyle. You will need to have a secure identity, psychological ‘sturdiness’, effective ability to cope, and willingness to open up to family support. The goal of the evaluation is to reflect upon these things with a professional to then receive certification that you are suited to take on such a challenge.
Pain Management / Spinal Cord Stimulator Procedures
Psychological characteristics play an important role in how individual differences in the pain experience may influence responses to SCS, as well as a variety of other pain treatments. For example, one key trait of someone who might cope well with the challenges of pain treatments is hardiness, or your overall attitude towards trials in your life. We will test your hardiness as well as other similar traits to make sure you are ready to proceed.
We may also review with you several techniques for managing pain and things that you can do to improve your health psychologically.
Evaluations for Police
There are a lot of demands placed upon law enforcement, and the life of a police officer can be emotionally, mentally, and physically difficult. There will be days when you are forced to stand firm yet polite in the face of tremendous verbal abuse, and there will be times when you are exposed to horrific scenes.
There are certain traits that police officers should have to be prepared for such demands. Traits such as impulse control, general intelligence, judgment, reasonable courage, honesty, integrity, and the ability to tolerate stress. We will also ask questions about prior drug use.
Concealed Handgun License
You can understand how someone who is enabled to carry must be of sound mind and morals. The process you will go through is simply to help us ascertain that you have a healthy head on your shoulders and that we can certify that you are an upstanding citizen. Our goal is for you to be able to show the state that you will do good with the certification you receive.
Hardships for Immigration
We are here for you to be here. We want to give you the proof of psychological distress, should it exist, or help you to achieve your goals and dreams here. As such, we try to make it as easy as possible. Translator’s are allowed and available. The intake questionnaire is also available in Spanish and English.
